SBAR Communication Framework
SBAR (Situation, Background, Assessment, Recommendation) is a structured communication framework widely used in healthcare to ensure clear, concise information exchange. Originally developed by the military and adapted for healthcare by Kaiser Permanente, SBAR is a frequently tested concept on the CPHQ exam.
The Four Components
Situation: State what is happening with the patient right now. This includes the patient's name, location, and the specific concern prompting communication. Background: Provide relevant clinical context, including diagnosis, pertinent medical history, and current treatment. Assessment: Share your clinical assessment of the situation, including vital signs, changes in condition, and severity. Recommendation: State what you think should be done, including specific orders or actions needed.
