What Is a Value Stream Map
A value stream map shows the flow of materials and information through a process from beginning to end. Unlike a simple flowchart, a VSM includes data about each step: cycle time, wait time, number of people involved, defect rates, and inventory levels. The map distinguishes between value-added steps (activities the patient would be willing to pay for) and non-value-added steps (waste). This distinction helps teams identify where to focus improvement efforts.
The Eight Wastes in Healthcare
Value stream mapping helps identify the eight types of waste, often remembered by the acronym DOWNTIME: Defects (errors requiring rework), Overproduction (doing more than necessary), Waiting (delays between steps), Non-utilized talent (underusing staff skills), Transportation (unnecessary movement of materials), Inventory (excess supplies), Motion (unnecessary movement of people), and Extra-processing (steps that add no value).
Creating Current and Future State Maps
The VSM process begins with creating a current state map that documents how the process actually works today, not how it is supposed to work. The team walks the process, observes each step, and collects data. After analyzing the current state for waste and inefficiency, the team creates a future state map showing the improved process. The gap between the two maps defines the improvement work to be done.
CPHQ Exam Applications
For the exam, know that VSM is particularly useful for complex, cross-functional processes such as patient flow through the emergency department, surgical scheduling, or discharge planning. It is a team-based exercise that requires participation from people who actually perform the work. VSM reveals systemic waste that may not be visible when looking at individual process steps in isolation.
