Force Field Analysis for Quality Leaders

Force field analysis is a change management tool developed by Kurt Lewin that helps quality leaders identify and evaluate the forces that support or oppose a proposed change. The CPHQ exam tests your ability to apply this tool in healthcare settings.

Understanding the Concept

Lewin proposed that any situation is maintained by a balance between driving forces (forces pushing toward change) and restraining forces (forces resisting change). The current state exists because these forces are in equilibrium. To create change, you must either strengthen the driving forces, weaken the restraining forces, or do both. Lewin observed that weakening restraining forces is often more effective and sustainable than simply increasing pressure for change, which can create tension and resistance.

Conducting a Force Field Analysis

Begin by clearly defining the desired change. List all the driving forces on one side and all the restraining forces on the other. Rate each force on a scale (typically 1 to 5) based on its strength. The visual layout creates a clear picture of the balance of forces and helps the team prioritize where to focus their efforts. The strongest restraining forces typically deserve the most attention because addressing them has the greatest impact on enabling change.

Healthcare Applications

In healthcare quality, force field analysis is valuable when planning major changes such as implementing a new electronic health record, redesigning a care pathway, introducing a safety reporting system, or consolidating services. Driving forces might include regulatory requirements, evidence of improved outcomes, or financial incentives. Restraining forces might include staff resistance, budget constraints, competing priorities, or technology limitations.

Integration with Change Management

Force field analysis works well in combination with other change management approaches. The insights gained from the analysis can inform strategies from Kotter's eight-step model or Lewin's unfreeze-change-refreeze framework. For the CPHQ exam, remember that force field analysis is both an analytical tool and a communication tool that helps build consensus among stakeholders about the challenges and opportunities associated with a proposed change.
